What was going wrong
They had ten different OSHA forms for ten different situations — daily check-ins, incident reports, equipment inspections, hazard flags. Each one lived in a different place. Workers in the field had to know which form applied, find it, fill it out, and get it to the right person. Predictably, forms got missed, misfiled, or filled out wrong. Audits were a multi-day scramble to piece together what actually happened.
The real problem wasn’t compliance — it was complexity. Ten forms is ten opportunities to pick the wrong one. The fix wasn’t more forms or better filing. It was one smart form that figured out what it needed to be based on what the worker selected.
